Calvin Ly, DO, MBA
Calvin is a diligent and ambitious individual who strives for quality and excellence; his goal is to deliver quality care to patients. He was valedictorian at Grace Davis high school, graduated with honors at UCSD in Biochemistry and earned the highest distinction of Summa Cum Laude at the Kansas City University of Medicine and Biosciences. Calvin earned his MBA in Health Care Leadership while attending medical school; his class was the second graduating class to take advantage of this dual degree program. His first career after Emergency Medicine training at Case Western Reserve University was with Kaiser in the Emergency Department in the Los Angeles and Orange County regions. He then transferred up to Northern California to take on a new leadership role managing the Cowell Health Center located at Santa Clara University. He honed his communication educational and management skills working with physician assistants, nurse practitioners, registered nurses and medical assistants, while developing memorable memories and rapport with the students at SCU. While working at Santa Clara University, Calvin kept up his emergency skillset by commuting up to San Francisco, working in the Emergency Department at the California Pacific Medical Center (both Davies and St. Luke’s Campuses) and at Chinese Hospital. He applied the Spanish he learned in high school and college and the Cantonese he learned as a child. He believes excellent communication skills are critical and crucial in delivering quality medical care to patients.
Calvin was born and raised in the Central Valley region of California. He is the second child of four children. His parents owned a computer business assembling custom designed desktops to clients, which included the Stanislaus Unified School District. He is the first generation in his family to obtain a four-year college degree. He is the first physician in the family. During his spare time, Calvin loves to be active: run, bike, hike, swim and play tennis and golf. During his down time, Calvin likes to read and watch comedies. Recently, he has been anticipating the new seasons of West World and the Game of Thrones.
Carolyn Seen, MD
After graduating from Dartmouth Medical School, Carolyn came to the Bay Area to do her internship/residency at California Pacific Medical Center. She has enjoyed a marvelous medical odyssey which has given her opportunities to practice in both the outpatient and inpatient settings. The breadth of this experience has resulted in a deepened appreciation for the social, emotional, physical and cultural nuances that make each patient’s story unique. Carolyn is an empathic listener who renews her goal each day to be better, to do better. While she marvels at the promise of emerging therapeutics and technologies to improve the quality of
people’s daily lives and to transform health care delivery systems, she remains firmly grounded in the “old school” truism, perhaps most famously stated by Sir William Osler, that “it is much more important to know what sort of a patient has a disease than what sort of a disease a patient has.” The approach of coordinating services in an accessible fashion is not only sensible, butnecessary. Thus, it is with great excitement that the adventure continues in the healing spaces at Crossover. Among Carolyn’s favorite mindfulness activities are: singing with the choir and practicing on her yoga mat. And, of course, for finding her center, she is found happily throwing at the potter’s wheel!
Jeff Peterson, MD
Dr. Jeff Peterson is an Emergency Medicine/Sports Medicine Physician with Primary Care experience. He received his B.A. from the University of California
at Berkeley and M.A. and M.D. from the Boston University School of Medicine. He completed a Surgical Internship, Emergency Medicine Residency, and Sports Medicine Fellowship at Stanford University Hospital. He also completed a Cardiovascular / Emergency Medicine Research Fellowship at the University of Colorado Health Sciences Center. From 2001 through 2016 he was on Clinical Faculty at the Department of Emergency Medicine at Stanford University Hospital in Stanford, California. Dr. Peterson has assisted in the medical care of several Bay Area Professional Sports Teams, professional athletes, and as a race physician and medical director for numerous endurance races across the globe. He served as the lone physician for the movie Running the Sahara, where three extreme athletes endured grueling conditions while becoming the first runners to cross the Sahara Desert. He was also honored to serve as physician for His Holiness the Dalai Lama during trips to San Francisco. He is committed to an integrative approach to medical care and is an avid triathlete, completing three Ironman triathlons and numerous Alcatraz triathlons.
Leslie Luff, BSN, RN
Leslie is an experienced Registered Nurse with over 20 years of health care delivery experience. She began her career at Stanford Hospital where she worked as a staff nurse, clinical nurse specialist, physician liaison, and PACU nurse. She later gained experience in several specialities including cardiology, orthopedic spine, gastroenterology, internal medicine, and ambulatory surgery. Leslie brings her love of serving and caring for others to her role as the practice manager, responsible for the day-to-day clinical management of the health center. She will help ensure your visit to the health center combines the highest quality of care with an exceptional experience.
Leslie is a California girl who was born in Malibu and raised in Menlo Park where she still resides today. She finds great satisfaction in working with people to help them reach their full potential. She is an active mom who enjoys baking, pilates, tennis, and is a huge football and baseball fan! She can often be found helping with her daughter’s crew team or trying to keep up with her son on a dirt bike.
Antoinette Shih, RN
Antoinette is a Registered Nurse who takes pride in providing holistic and quality care to all of her patients and families. She is a compassionate and driven individual who strives to perform comprehensive clinical practice in the healthcare setting while building relationships both on a professional and personable level. She believes that proper communication and patient education is essential in achieving individualized health goals. Antoinette graduated from the Dominican University of California with Honors and joins the Crossover team with a background as a Recovery Nurse in the perioperative setting, a Home Health RN Case Manager as well as experience as a clinic nurse at a fast paced multi-physician OBGYN office. Antoinette is eager to collaborate with the multi-disciplinary team in preventative healthcare.
Antoinette was born and raised in the San Francisco Bay Area with a passion for traveling and experiencing new things. Leading a healthy and active lifestyle, Antoinette enjoys working out through various fitness activities. With past experience as a stylist, she also has a strong appreciation for style and fashion. In her spare time, she enjoys listening to a broad range of music, reading, and creative writing, and values time spent with family and friends.
Tami Leung, RN
Tami is an energetic and compassionate individual who takes prides in providing excellent customer service and patient care. She earned her nursing degree from California State University, San Marcos. She has worked in various health fields such as home health, oncology, intensive care, and public health. Tami had the opportunity to work with the community and local hospitals in Vietnam where she fell in love with patient education and preventative health care. Before her nursing career, she worked in the business industry and earned her Marketing degree from the University of Michigan.
Tami is a Michigan native and moved to sunny California in 2008. Since then, she has settled her roots in the south bay and enjoys yoga/pilates and spending time with her family and friends.
Rachel is a Bay Area transplant who grew up in the suburbs of Long Beach, CA. She graduated from San Francisco State in 2012 where she earned a Bachelors Degree in Anthropology. Her intrigue & passion for human history created a space to fill with knowledge of evolution, culture, and the future of civilization as we move forward with technology.
Her professional career has gravitated more towards fast paced environments where she can learn many new skills while helping others. When Rachel isn’t working as an ambassador for the health center, she is hiking with friends, traveling the world with her partner, and creating unique jewelry using precious metals, stones, and organic materials.
Marybeth is an upbeat and dedicated individual who strives for growth and excellence. She loves communicating and working with people, and highly values building relationships. She is reliable and thrives on learning and expanding her knowledge in from experiences. Marybeth has demonstrated her communication skills in many different avenues. She worked in the retail and student learning industry for over three years. During that time, she learned to balance many tasks while attending school and build trusting relationships with customers from diverse backgrounds. During college, she also interned with Planned Parenthood Mar Monte, a nonprofit organization that provides medical and education services to several communities. She worked closely with the Volunteer Services and Human Resources Departments and enjoys volunteering whenever she can.
Marybeth was born in San Jose, CA and raised in Manteca, CA. She attended college at San Jose State University where she received her Bachelor’s degree in Health Science with a concentration in Health Services Administration. In her free time, she enjoys watching watching movies, hiking, and spending time with her family and boyfriend. She also spends as much time as she can spoiling her energetic Pitbull and Chihuahua mixed dog named Spike.
Elaine is a highly dedicated and compassionate individual who strives to make a positive impact on the health and well-being of others. Elaine graduated with honors from San José State University where she received her bachelor’s degree in Health Science. She has worked in the healthcare field for years and has prior experience in patient satisfaction and community health outreach. Her previous positions at a non-profit cancer research organization and Stanford University’s Asian Liver Center has allowed her to promote health and wellness among a range of people.
When Elaine isn’t working, she is spending time with her loved ones, exploring the great outdoors, or mixing up a batch of desserts in the kitchen. She also enjoys traveling and hopes to be able to visit as many destinations as she can in her lifetime. She’s up for any adventure and is excited to help patients venture on their own personal wellness journey here at Crossover.
Conrad Young, PT
Conrad graduated from Physical Therapy School in Auckland, New Zealand and has subsequently completed additional training in the 18 month long Folsom Physical Therapy Manual Therapy training, completed mutliple parts of the McKenzie method for mechanical diagnosis, training in the Ola Grimsby Institute Long Term training, become a Certified “Whole Body” Active Release Technique Practitioner, and attended multiple short courses on everyting from neural mobilization techniques to sports conditioning. Conrad has also been part-time faculty of the Auckland Institute of Technology Sports Medicine Dept as well as the Hayward Kaiser Long-term Residency program.
Outside of his occupation, Conrad is an avid multi-sport participant especially with Triathlons. He has run everything from Sprint to Ironman distance races. He is also a USAT Certified Triathlon Coach, Vice President of the Palo Alto Run Club, and a general fitness enthusiast. One usual interest is his special skill as an apiarist (a fancy word for a beekeeper) which he maintains on the weekends.
Naroth Mendoza, DC
Dr. Naroth is an alumni of Life Chiropractic College West, in Hayward, CA. At LCCW, she received her Doctor of Chiropractic Degree with Cum Laude, and has been serving her community for the past decade. During her last year in school as a student doctor, she was able to go on a medical trip to San Salvador, El Salvador, where she was able to serve the people and provide chiropractic care and education. Since then, she has continued her humanitarian work and took another trip to El Durazno, Guatemala, where she built stoves to improve the lives and respiratory health of the people living in the village. She continues to help improve the health of her community and has been focused on working with athletes and women’s health. She is certified in Rock Tape and the Graston Technique. She currently resides in the San Francisco area and enjoys all her extra time running around with her two children. She keeps active with her obsession with barre3 and weight lifting when she is not with her children.
Brendon Rearick, CFSC
Brendon’s personal mission is to spread the positive by-products of coaching and movement by using strength and conditioning as his platform. As a member of the Crossover care team, he is on a mission to make physical activity the new “most commonly prescribed drug.”
With a degree in Kinesiology from the University of Massachusetts Amherst, Brendon got his start at Mike Boyle Strength and Conditioning (MBSC) in Woburn, MA. The growth-focused environment at MBSC motivated Brendon in a way he had never experienced. Over a few short years he progressed to become the Program Director and earned his massage therapy license. Soon thereafter, he started an education company with four co-workers – Certified Functional Strength Coach. In just three years, CFSC has certified over 3,000 trainers in over 20 countries.
Now a California resident, Brendon travels and coaches often for Certified Functional Strength Coach, and has recently become an active speaker at the Perform Better summit conferences. With an inexhaustible drive to learn more, practice his craft, and be the best version of himself, Brendon dedicates his free time to continuing education, dancing with his 3-year old daughter, reading, writing, and enjoying in the outdoors.
Allison developed a passion for strength and conditioning when she started training to play Division 1 field hockey at Quinnipiac University located in Hamden, CT. While she attended school she was diagnosed with a brain infection that changed her career path and life plan. She began to realize that health should be a priority and that taking care of your body can sometimes be the best form of medicine.
She earned her degree in Health Sciences from Quinnipiac University in May of 2017 and completed several internships before earning her CSCS in 2018. She also has an FMS certification.
Allison believes that exercise is much more than just getting strong or getting fit. She believes that exercise can help improve health status, movement patterns, prolong life expectancy, as well as build confidence.
Outside of the gym, Allison loves to continue to educate herself as much as possible by reading and spending time with other fitness professionals. She also enjoys going to the beach and traveling.
Rachel Combs, LAc
Rachel was born and raised in Reno, Nevada and has made Santa Cruz her long time home. She obtained her Bachelors of Science from UC Santa Cruz and later earned her Masters in Traditional Chinese Medicine from Five Branches University also in Santa Cruz. Rachel’s health care philosophy is firmly rooted in compassion and cultivated by a love of modern and ancient medical knowledge. Rachel’s clinical expertise in the safe and effective use of acupuncture techniques is matched by her previous experience working in an integrated care environment. She believes this is the best environment to help patients get well and stay healthy.
Rachel’s life philosophy includes giving back, and she regularly teaches on topics like mind / body balance and stress reduction while still volunteering free acupuncture services for organizations like Women Care and Santa Cruz Homeless Shelter. She has a strong love of nature, friends and family with the resulting belief that being outdoors is an important addition to a balanced, happy, and meaningful life.
Born and raised in the south bay area Jennifer received her BA from Humboldt State University before deciding massage was the path she wanted to follow. Jennifer is a Nationally certified massage therapist and has been practicing massage for 20 years. In those years she has over 1000 hours of training as well as being trained as a Pilates instructor in the Polestar method.
Jennifer designs each session based on client goals and needs. Whether you completed your first Spartan race, need to de stress or just want to have the kinks worked out, Jennifer can provide a massage to help you reach your goals. Jennifer utilizes many techniques including deep tissue, cranial sacral, myofascial release, and cupping. Jennifer believes that how we take care of ourselves today influences how we age.
Outside of work she spends time in the gym, playing with her three dogs and cooking meals from her garden.
Maxine Yeung, MS, RD, CPT
Maxine is a Registered Dietitian, Certified Personal Trainer and Health Coach. Her interest in nutrition started with her first career as a pastry cook. Passionate about cooking, she longed to have a better understanding of how different foods affect the body. She pursued her curiosity, completing her dietetic internship and masters in clinical nutrition through New York University, and worked in a variety of settings from hospitals to gyms. Speaking with Maxine, you quickly learn she is truly client-centered and focuses on a positive mindset. She believes in a balanced lifestyle and is dedicated to helping you find a realistic way to achieve your health goals.
Maxine was born and raised in Maryland and moved to the Bay Area seeking access to nature after living in New York City for nine years. She loves anything outdoors, especially stand up paddling, hiking and running, and dreams of becoming a ski bum. When she is indoors, you will find her baking, crafting and spending time with her family.
Ross Nelson, PsyD
Ross obtained his bachelors in psychology from Santa Clara University and later his Doctorate in Psychology at the California School of Professional Psychology in San Francisco. Ross completed both his pre-doctoral internship year and post-doctoral residency at Kaiser Permanente. He then went on to work for Kaiser for an additional 2.5 years where he continued to work amongst an integrative team supporting adult clients. He is interested in research-based interventions and has developed a specialty in treating anxiety, improving cognition, increasing performance, and reducing stress.
Ross was born and raised in San Diego, but has made the Bay Area his home for the past 14 years. He considers himself a “foodie,” enjoys playing as well as watching various sports, being outdoors, and spending time with his wife and friends. As an extension to his clinical practice Ross is experienced in guided mindfulness practices, interpersonal effectiveness, and integrating tech opportunities with client services.
Nina Kalus, PsyD
Nina is a licensed clinical psychologist who is dedicated to supporting people’s growth, both individually and in relationship to others. She received her Bachelor of Arts in psychology from Vassar College in Poughkeepsie, NY and her Doctorate of Psychology from the California School of Professional Psychology in San Francisco, CA. Nina has experience working with individuals, couples, and families in hospital, school, residential, and community mental health settings. Her specialties include relationship difficulties, adjustment issues, stress, anxiety, and depression.
Nina grew up in the city and suburbs of Chicago and has been living in the Bay Area since 2010. She enjoys cooking, traveling, movies, barre classes, and hiking. Alongside her husband, Nina loves exploring all that northern California has to offer.

Annie Low, OD
Annie is a primary care optometrist who has been in practice for five years. She began her career in a small private practice and has continually expanded her practice settings by moving to a large retail setting and then to the employer health industry. She has continued to develop her patient skills as well as learned how to successful manage the business side of optometry in these various settings. Her practice scope ranges from infants and geriatrics and everything in between. She has employed by Crossover Health for several years and appreciates the opportunity to practice in an integrated care environment. She is excited to help launch the new Mountain View Practice.
Annie grew up in the suburbs of Chicago, but lived in the East Bay for almost 10 years during her undergraduate years and as she began her career in optometry. She loves the California sunshine and enjoys her free time with her husband and twin sons. She is a voracious reader (2-3 books per week), a music aficionado who can play six instruments, and has the dream to one day produce an album.
Anastasia has a gift for relationship building which she has honed in a variety of clinical settings over her working experience. In her role as host, Anastasia is the first person to greet you and help you feel welcome as you begin the care experience. She also helps with any patient service issues, including coordinate appropriate follow up. She was fortunate to work and gain experience while in various medical clinics ranging from a surgery office to a physical therapy clinic. Anastasia is a San Francisco native and a graduate of St. Mary’s College of California.
Anastasia loves San Francisco and continues to enjoy exploring and finding hidden gems the the city has to offer. She has a goal to visit every stadium in the NFL and MLB. She has a passion for sports, including both the Giants and 49ers. She previously served as a 49ers Fan ambassador where she provided game day customer service for 5 seasons. When not working she enjoys traveling and hanging with friends and family.
