If you choose to send a message, a care team member will respond within 2 hours of receiving your message. You will get an email alert to log back into the secure platform to read the response. Continue messaging with your care team until your issue is resolved.

Come to us for any health concern – big or small. As your medical home, we manage all aspects of your health from wellness check ups, minor injuries, and sick care to chronic health problems like diabetes or high blood pressure.

Keep a healthy back and spine. Reduce pain fast, increase mobility, and strengthen your posture.

Mental Health

Cope with life’s troubles and learn to thrive even in uncertain or difficult times. A healthy body includes a healthy mind.

Health Coaching

Get help with sleep issues, meet fitness goals, or learn to eat healthier. We can also help you quit smoking, find better work-life balance, and stay at a healthy weight.

Acupuncture

Treat anxiety, stress, insomnia, back and neck pain, joint disorders, and muscular complaints with acupuncture. Our acupuncturists actively coordinate with other Crossover providers to ensure an integrated approach to your care.

Annual Physicals

A yearly exam helps ensure good health throughout the year and for years to come.

Prescriptions

You can pick up medicine refills at your local pharmacy or have them sent to your home. We can also answer any questions you may have about your medicine.

Care Navigation

Your Care Navigator will be your guide from start to finish for any appointments you need outside of Crossover.

Labs

Work with your doctor to get blood draws and diagnostic tests to help guide your care plan.

Vision

The VSP dispensary can assist you with eyeglass/contact lens orders, eyewear adjustment and minor repairs.

Common Questions

If you have received care with Crossover at the Life@ Wellness Center at MPK or in ATX in the past, you should already have a Crossover account and can log in using the same email and password. Once you login in and to start your care, follow the steps below:

- For virtual care click on “Start a Conversation” and follow the prompts to describe your health concern or question.

- For in-person care at midtown click on “Schedule a visit” and follow the prompts to find an appointment time that works for your schedule.
There are no fees or limits for messaging your care team and your provider. Some health concerns can be solved this way and will not require an appointment. In the instances where a scheduled video or phone visit is needed or preferred, a copay or coinsurance will be charged similar to an in-person visit.

For in-person appointments at the Crossover Health Center in midtown, care will follow your regular fee schedule and copays and coinsurance apply.

For all outside lab orders and in-person community referrals, you will be billed according to your medical plan design.
You can message your Crossover care team anytime, from anywhere in NY, NJ or CT and receive a response typically within 2 hours or less, during business hours (Monday - Friday 8am-5pm ET).

If you log in after business hours and start a conversation that requires attention, you will be given the option to escalate your care to “urgent” by calling the phone number shown on screen to be connected to Crossover’s on-call physician.

You can schedule a virtual or in-person visit with Crossover Monday - Friday, 8am to 5pm ET.

You can message a primary care provider 7 days a week between 8am-5pm ET.
Although response times may vary, the care team will typically respond to a “Conversation” and any subsequent messages, within two hours of receipt during business hours Monday - Friday 8am - 5pm local time. This allows you to go about your day without finding and scheduling an appointment, commuting, sitting in a waiting room, and repeating the sequence with any needed follow-up appointments.
