JUST ACCESS

$65 per month membership fee

For your peace of mind all year long. The Just Access package includes an annual physical examination, plus access to our centers as needed with any of our Crossover providers for 12 months. Visits will be charged at out-of-pocket prices provided in the FAQs below.

JUST THERAPY

$95 per month membership fee

A Just Therapy membership package allows you to work with our care team to treat an injury, chronic pain, or an emotional/behavioral health issue. Members can schedule with Crossover’s Physical Medicine (Physical Therapists, Chiropractors, Acupuncturists, Massage Therapists) or Behavioral Health (Psychologist, Licensed Social Worker, Health Coach) professionals for an allotted 8 visits per year, with full membership access for 12 months to schedule visits with Crossover providers as needed at out-of-pocket prices (provided in FAQs).

This package includes our Virtual Fitness subscription at no charge (a $65 per month value).

PRIMARY HEALTH

$125 per month membership fee

A Primary Health membership package provides a board-certified PCP for your annual preventive check-up (plus labs) and sick visits, immunizations, or prescriptions needed outside of your initial primary care appointment. Members can schedule with a primary care physician for an allotted 3 visits per year with full membership access for 12 months to schedule visits with Crossover providers as needed at out-of-pocket prices (provided in FAQs).

This package includes our Virtual Fitness subscription at no charge (a $65 per month value).

What a Crossover Membership means for you
FREE TO TAKE YOUR TIME Typically, healthcare providers can only do what will be reimbursed, and volume is encouraged. We do things differently. Our care teams aren’t incentivized to overbook their calendar with patients so that you get time to build the relationship that works for you.

EVERYTHING’S CONNECTED We bring doctors and specialists together under one roof. For example, in our practice, your doctor is able to consult with you physical therapist, or your health coach and mental health specialist are able to discuss how your antidepressant may be affected by your diet.

ACCESS IS EVERYTHING Inconvenience shouldn’t be a reason to skip a trip to the doctor. We’ve built modern care centers near you, that offer wait times under five minutes. And if traffic, meetings, or your condition are a challenge to the care center, you can schedule virtual care appointments.

  • Is there an age minimum to become a Crossover member?
    Yes, all members must be at least 18 years of age.
  • Which services are covered in a Crossover Membership?
    Different services are available in different packages so you can choose the one that best suits your needs.

    The Access Package includes one appointment of the following services
    Primary Care: routine office visits with a physician, urgent care visits, episodic care, and travel medicine (this includes all lab tests and immunizations)

    Preventative Care: annual physical or annual well-woman exam (this includes all lab tests and immunizations)

    The Therapy Package includes a Virtual Fitness subscription and 8 appointments of the following services:

    Specialty Care:

    Health Coaching

  • What is Virtual Fitness?

  • Which services are not covered in a Crossover Membership?
    Optometry

    Personal training
    Medications: We can dispense 80% of the most commonly prescribed medications in our office for a fee of $5.00 each. For any medication we do not carry, a prescription can be written or electronically sent to the pharmacy of your choice. When you pick your prescriptions up at a pharmacy, you will need to bring your insurance card, and you may be responsible for any co-pays. If you prefer to have your medication sent directly to your home, we can sign you up for our mail order pharmacy program. As a reminder, you may also be responsible for a co-pay when using the mail-order pharmacy option.

    Glasses and Contacts: We have onsite VSP-run stores in all of our clinics where you can purchase frames or contacts. If you have VSP insurance you can use that towards your glasses or contacts.
  • How do I determine which package is a better fit for me – Access, Therapy, or Primary Health?
    It all depends on the amount and type of care that you think you will want or need throughout the year. Take into consideration your health goals and how you prefer to interact with your healthcare providers.

    Access - A membership package for your peace of mind. Includes an annual preventive check up and full membership access to schedule visits with Crossover providers as needed throughout the year at out-of-pocket prices.

    Therapy - A membership package to work on my injury, chronic pain, or an emotional/behavioral health issue. Members can schedule with Crossover's Physical Medicine (PT, Chiro, Acu, Massage) or Behavioral Health (Psychologist, Licensed Social Worker, Health Coach) professionals for a treatment plan and follow-ups.

  • What if I want to use a service that is not covered in my membership?
    You can! You will just need to pay for them in addition to your monthly fee.

  • What happens if I go over my allotted number of visits?
    Extra visits are simply paid for out of pocket. Here is the fee schedule:
    Preventative Care: $250
    Primary Care: $250
    Chiropractic Care: $150
    Physical Therapy: $150
    Acupuncture: $150
    Health Coaching: $150
    Behavioral Health: $150
    Massage: $100 (for 50 minutes)
  • What if I change my mind about my membership?
    If you would like to cancel your membership you can do so at any time by emailing us at membership@crossoverhealth.com or submit written notification to one of these locations (crossoverhealth.com/locations-bay-area). Your membership will then terminate on the last day in your current monthly billing cycle. The monthly fees paid will not be refunded or prorated.
  • Can I use my HSA/FSA dollars to pay for membership or additional services purchased?
    No. Since personal memberships do not work through insurance, we cannot accept HSA/FSA funds as payment.
  • When does my membership start and stop?
    It starts on the day you complete the enrollment process and ends 1 year from that date.
  • What if I change my mind about my membership?
    If you would like to cancel your membership you can do so at any time by emailing us at membership@crossoverhealth.com or submit written notification to one of these locations (crossoverhealth.com/locations-bay-area). Your membership will then terminate on the last day in your current monthly billing cycle. The monthly fees paid will not be refunded or prorated.
  • Where is Crossover Health located?

  • What are the Crossover Health office hours?
    All of our clinics are open from 8 am to 5 pm, Monday through Friday. Members who find themselves with an urgent issue outside of business hours can access our on-call physician (this is not for medical emergencies).
  • Can I have unlimited communication with my provider?
    Members can message their care team directly via our online patient portal an unlimited amount of times. However, if a question or request is considered too complicated to be addressed in writing, your provider may ask that you schedule an appointment. We typically respond to portal messages within 24-48 business hours.
  • Can Crossover Health be my primary care provider?
    Yes. However, because we don’t work with insurance, we will not be able to be listed as your PCP on your insurance plan.
  • What are the Crossover Health office hours?
    All of our health centers are open from 8am to 5pm, Monday through Friday. Members who find themselves with an urgent issue outside of business hours can access our on-call physician (this is not for medical emergencies).
  • Does Crossover Health have specialists?
    No, our physicians are general practitioners. However, we can refer you to any specialists in your area and coordinate with them on your follow up care.
  • Does my monthly fee include the cost of any necessary labs?
    Yes.

    *Please Note: In New York, it is state law that any labs sent to a lab vendor for processing have to be submitted to insurance and therefore could have a patient cost associated with them. Any labs/tests that can processed in-house are included in the membership fees.

  • Does my monthly fee cover the cost of medications?
    No. Please see question #2 for details on how members can receive medication, and what the related costs and requirements will be.
  • Does my monthly fee cover the cost of any visits outside of Crossover Health Centers?
    No.
  • Does my monthly fee cover the cost of imaging (X-Ray, MRI, CT Scan, etc.)?
    No. The membership fee only covers services within a Crossover Center.
  • What if I have an emergency?
    If you are having a medical emergency you will need to call 9-1-1 or go to the Emergency Room. If you have medical insurance, your visit will be billed accordingly.
  • Do my Crossover Health visits count towards my deductible or out-of-pocket max?
    No. Membership with Crossover Health is separate from and unrelated to a member’s insurance policy, so no claims will be submitted to insurance.
  • How long is a membership contract with Crossover Health?
    Twelve months
