Primary care, physical medicine, mental health, and health coaching are all available with both in-person and virtual care. Your hybrid healthcare team will work together to make sure that the care you are receiving is connected and seamless with no extra run-around. When you need in-person care outside of Crossover such as imaging or specialist care, our Care Navigators will help you make an in-network appointment in your community and coordinate all of the paperwork.
