You can earn $100 each quarter by completing and reporting (as needed) at least one wellness activity* through the WellRight app.

To qualify for an additional $400 annual payout, you must:

  • Complete and report (as needed) all four quarterly wellness activities, and
  • Complete and report (as needed) an annual physical and/or biometric exam

Crossover makes it easy to achieve your wellness goals and earn incentives. Schedule an annual exam or biometric screening with your provider to get started.

*Completing and reporting (as needed)  more than one activity per quarter will not increase the incentive reward.  Only one completed activity is required—and counted—for the quarterly $100 reward.

**Biometric screenings held in October-December will be reserved for newly hired associates throughout the year to ensure there are adequate opportunities for everyone.

Here are some examples of activities you can complete to earn your quarterly $100 incentive:

  • Complete a free Fitness Assessment with a Crossover Fitness Coach
  • Complete a virtual ergonomic assessment with a Crossover Physical Therapist
  • Get a flu shot with Crossover or a local pharmacy / provider
  • Participate in the Crossover Fitness Team’s 28-Day Challenge
  • Listen to a BeWell podcast or attend a Crossover wellness webinar
  • Participate in an athletic event: 5K, charity walk, bike race, etc.

How the Live Well Wellness Incentive Program Works

Quarterly Incentives

To be eligible for the $100 quarterly incentive, you must:

  • Complete and report (as needed) one eligible activity per quarter (see examples above)
  • Be actively employed on the first day following the quarter (see dates)
Quarterly Submission Dates

Complete and report (as needed) the quarterly wellness activities:

  • Q1: January 1 – March 31
  • Q2: April 1 – June 30
  • Q3: July 1 – September 30
  • Q4: October 1 – December 31
Annual Incentives

To be eligible for the $400 annual incentive bonus, you must:

  • Complete and report (as needed) all four quarterly activities (see dates)
  • Complete and report (as needed) an annual exam or biometric screening by December 31st
  • Be actively employed on the first day of the following year

Register Your New Live Well Account Today

  1. Activate Your WellRight Account
    You can either:
    • Login to or register your Crossover account at care.crossoverhealth.com, and click on the Live Well Program tile on your Dashboard to get started.
    • Go directly to jacksonhealthcare.wellright.com, and click “Register” towards the bottom of the login box.
  2. Complete the Registration Form
    Select “Employee” and fill out the registration form using your legal name (first and last) and your work email address.
    • If asked for group code, use: jacksonhealthcare
  3. Verify Your Email
    Check your inbox for the Email Verification message and click the link to confirm your work email address.
