All Frost employees enrolled in a medical plan (Frost or non-Frost medical plan) are eligible for Crossover Health services. Adult dependents enrolled in a Frost medical plan are eligible for the same care services with Crossover. Because One Frost is a secure campus, the Frost Wellness Center is available to employees only at this time. Adult dependents may access Crossover Health Centers or Crossover’s virtual services.

All of your Crossover providers work together to help you get and stay healthy. You’ll connect with the same Care Team every time regardless of whether the care you receive is virtual or in-person. Your team includes a Care Navigator who will personally guide you when you need to access care outside of Crossover.

For example, if your Crossover Primary Care provider recommends a visit with a dermatologist, cardiologist, or surgeon, your Care Navigator will identify a top local specialist in your Frost medical plan network and help you make an appointment.

The Care Navigator will also help arrange tests and lab work and ensure Crossover has your results.

No. Crossover is simply a healthcare provider offered under the Frost medical plan.

Yes, the choice is yours. It is not necessary to disrupt an established relationship you have with a quality care provider in the community. However, we encourage you to utilize any services at Crossover that fit your needs.

Crossover members who leverage multiple services provided at our health centers appreciate the convenience, quality, and cost savings of our coordinated Primary Care model. About 80% of Frost employees have made Crossover their primary care home.

Your Crossover Care Navigator will ensure you have support connecting to services available at Crossover and help you easily access whatever care you may need in the community outside of Crossover.

They’ll identify a top local specialist in your Frost medical plan network, help you make an appointment, transfer records back and forth, and follow up with you to ensure you receive the care you need.

• In-network specialist referrals
• Appointment scheduling
• Records transfers
• Labs and imaging coordination
• Immunizations and screenings
• General care support

During the process of creating your account you will also agree to the terms of use, provide your consent for treatment, and be advised as to your privacy rights and how your personal health data will be used.

Once you have activated your no-cost platform account, you are able to access it from ANYWHERE a few different ways:

• Using our mobile app: You can download the mobile app from the Apple App Store or Google Play, just search “Crossover Health”.
• Using your web browser: Visit care.crossoverhealth.com to go directly to the platform sign in page or visit crossoverhealth.com/frost and click “Sign In” in the top right corner.

Our Care Team and physician are on call for the health center 24/7. Our providers are available to help triage your phone call and direct care as appropriate based on your need. For after hours assistance, please call the health center at (888)-854-1347.

If you need care outside the Health Center, our Care Team will help coordinate. We can help recommend specialists as well as assist with scheduling an appointment. We also work with you to ensure that all the information from outside care is collected back into your medical record so it is complete.

• One of the reasons to activate your account with Crossover is to take advantage of the personal health record, which can serve as a repository of your health information. The technologies involved are constantly evolving and the capabilities to store data are expanding as rapidly as the devices, apps, and related tools are in capturing new sources of data.
• Your personal health information is securely stored by the electronic health record system utilized by Crossover Health. Internet security protocols, privacy protections and encryption technologies that meet health care standards are utilized.

No. A fundamental term of use and central tenant of your privacy rights are that your employer will never have access to your personal health information. Your consent for treatment is between you and your provider who belongs to an independent medical group.

• Frost Wellness Center: Frost has selected Crossover Health Medical Group as their partner to manage the Frost Wellness Center. You can learn more about the Frost Wellness Center providers by logging into your account at care.crossoverhealth.com, clicking on “My Care Team” on your dashboard, and then selecting the different health centers to view the providers bios.

• Yes! The Frost Wellness Center and Crossover Health Centers are staffed by physicians that offer comprehensive Primary Care to include, acute care issues, chronic disease management (diabetes, thyroid, blood pressure monitoring), prevention and wellness checkups, men’s health, women’s health, minor procedures, immunizations, travel medicine etc.)
• The Frost Wellness Center also provides other services such as chiropractic care, physical therapy, mental health and health coaching.

• If you need a prescription, the physician can electronically send the prescription to the pharmacy of your choice. Our Care Team is available after hours, including nights, weekends, and holidays. For after hours assistance, please call the health center at (888)-854-1347.
• Once you activate your Crossover account, you will be able to request a refill of prescriptions online. Depending on the nature of the request and medication involved, the provider will access your electronic health record to electronically refill for pickup at your pharmacy.
