Schedule an in-person appointment at Crossover’s locations

Send a message about your health concern or question to your care team

If you choose to send a message, a care team member will respond within 2 hours of receiving your message. You will get an email alert to log back into the secure platform to read the response. Continue messaging with your care team until your issue is resolved.

Primary Care

Come to us for any health concern – big or small. As your medical home, we manage all aspects of your health from wellness check ups, minor injuries, and sick care to chronic health problems like diabetes or high blood pressure.

Physical Therapy

Heal aches and pains in your bones, joints, and muscles so you can move around more easily.

Chiropractic Care

Keep a healthy back and spine. Reduce pain fast, increase mobility, and strengthen your posture.

Mental Health

Cope with life’s troubles and learn to thrive even in uncertain or difficult times. A healthy body includes a healthy mind.

Health Coaching

Get help with sleep issues, meet fitness goals, or learn to eat healthier. We can also help you quit smoking, find better work-life balance, and stay at a healthy weight.

Acupuncture

Treat anxiety, stress, insomnia, back and neck pain, joint disorders, and muscular complaints with acupuncture. Our acupuncturists actively coordinate with other Crossover providers to ensure an integrated approach to your care.

Prescriptions

You can pick up medicine refills at your local pharmacy or have them sent to your home. We can also answer any questions you may have about your medicine.

Vision

The VSP dispensary can assist you with eyeglass/contact lens orders, eyewear adjustment and minor repairs.

Common Questions

Register at care.crossoverhealth.com. Once you login in and to start your care, follow the steps below:

For virtual care click on “Start a Conversation” and follow the prompts to describe your health concern or question.

For in-person care click on “Schedule a visit” and follow the prompts to find an appointment time that works for your schedule.

Initiating care and messaging back and forth with a provider is free. Scheduled in-person, video or phone visits, will be charged a copay.
There are no fees for starting a “Conversation” with your care team and no limits on messaging back and forth with your provider. Some health concerns can be solved this way and will not require an appointment. For any in-person, video or phone visits, care will follow your regular fee schedule. For all outside lab orders and in-person community referrals, you will be billed according to your medical plan.
Providers from each service line are available from 8am-5pm local time Monday through Friday. Virtual Primary Care is available 7 days/week 8am-5pm local time. If you log in afterhours and start a conversation that requires attention, you will be given the option to escalate your care to “urgent” by calling the phone number shown on screen to be connected to Crossover’s on-call physician.
Although response times may vary, the care team will typically respond to a “Conversation” and any subsequent messages, within two hours of receipt during business hours Monday - Friday 8am - 5pm local time. This allows you to go about your day without finding and scheduling an appointment, commuting, sitting in a waiting room, and repeating the sequence with any needed follow-up appointments.
All benefits-eligible employees and your covered adult dependents (ages 18 and up) enrolled in the UnitedHealthcare HDHP with HSA plan are eligible.
