Crossover Health provides Advanced Primary Care via a collaborative, holistic, flexible approach that brings Primary Care, Health Coaching with Registered Dietitians, Pediatrics, Care Navigation, and Occupational Medicine together to deliver better experiences and industry-leading outcomes. Available through in-person and/or online care channels, Crossover allows members to develop an ongoing, trusted relationship with their dedicated, connected Care Team. Crossover’s providers meet members where they are on their health journeys, and address members’ health history, environment, and the unique circumstances that affect their overall wellbeing. Providers and members work together to identify, set, and achieve health and wellness goals. If a member needs care outside of Crossover, they help every step of the way. Crossover sees health as it should be—connected and without barriers.

Crossover’s flexible approach makes it easy for you to get quality care and connect with their passionate team of providers through multi-channel access: online via messaging and video visits, over the phone, or through in-person appointments at the Gwinnett Employee Wellness Center.

Start by signing up for a Crossover account. Once you become a Crossover member, you get access to Crossover’s team of providers at the Gwinnett Employee Wellness Center, including a Family Medicine Physician, Medical Assistant, Health Coach who is a Registered Dietitian, and Care Navigator. There are no complicated bills, insurance submissions, or unnecessary paperwork. The care you receive from Crossover will be no to low cost. Primary Care: Get an annual exam, sick care, preventive screenings, request labs, and order prescriptions. Plus, get help managing long-term conditions like diabetes and high blood pressure.

Health Coaching: Work with a Registered Dietitian who can help with nutrition, fitness, sleep issues, and setting and achieving your goals for a healthy lifestyle. Crossover’s Health Coaching also includes support to manage weight, improve work-life balance, and quit smoking.

Select Pediatric Care: Get annual exams, sick care, and sports physicals.

Care Navigation: A virtual Care Navigator will be your guide from start to finish for any services or appointments you need outside of Crossover.

Prescription Orders and Refills: Prescriptions can be sent straight to your local pharmacy. Some medications may be able to be filled at the health center by your provider.

Occupational Medicine: Easily take care of any required work fitness exams (pre-employment, fit-for-duty, return-to-work) or medical surveillance services like drug testing.

Crossover’s dedicated teams of healthcare professionals deliver comprehensive, coordinated care—in person and virtually at the Gwinnett Employee Wellness Center. Crossover’s secure technology connects you to the team, and the team to each other. The team includes a Board-certified Family Medicine Physician, Nurse Practitioner, Health Coach who is a Registered Dietitian, and Care Navigator.

We understand that everything is connected in terms of your overall wellbeing. With the Crossover team, you see the same Care Team every time. Your providers work together to get to know you and meet your health needs. By using a collaborative team of providers across clinical disciplines, the Care Team helps close care gaps that siloed, single-specialty providers may miss.

All your providers work together to help you get and stay healthy, and you see the same Care Team every time you need it, whether in-person or virtually. You can send messages to your Care Team and they can answer your questions, all in the Crossover Platform or app. In addition, the Crossover Care Team includes a Care Navigator, who will personally guide you when you need to access care outside of Crossover. The Care Navigator will also help arrange tests and lab work, and make sure your records are transferred back to Crossover.

Your Crossover Care Team can provide you or your community-based provider with a copy of your Crossover medical records if the appropriate consent documents are signed. You can limit the amount and types of information that you share (e.g., medication list, medical bills, lab results, consultation results, etc.). Crossover can also receive any medical records from your community-based providers.

The Crossover Care Navigator will help coordinate referrals to care specialists outside of Crossover, including referrals to surgery, imaging, and other specialty services. The Care Navigator will help identify high quality, in-network specialists, assist with prior authorizations, and help close the loop with the referring Primary Care provider.

Yes, you can use Crossover providers for some services and other providers for other services depending on your health needs.

Yes, you can use Crossover’s provider as your Primary Care provider. This may consist of routine preventive exams in addition to chronic condition management. If you do not wish to use a Crossover provider as your Primary Care provider, you may still receive Health Coaching with a Registered Dietitian at the Gwinnett Employee Wellness Center for other health needs.

Yes. The choice is yours. You are not required to switch your primary doctor to use Crossover services. At your request, your records from your Crossover Care Team can be sent to your community healthcare provider or vice versa.

Your Crossover Health Coach, as a Registered Dietitian (RD), is equipped with the expertise to guide you toward optimal health and wellness through personalized nutrition plans and lifestyle adjustments. They view food as a fundamental component of your overall well-being and understand its potential to serve as medicine in supporting your health goals.

Whether you’re aiming for weight management, seeking sports nutrition advice, looking for meal-planning guidance, or navigating specific health concerns or diagnoses, your Crossover Health Coach is your advocate. Direct message: You can send a direct message to your whole Care Team, or to any specific member of your Care Team with whom you’ve had previous interactions.

When signing up for a new account, providing an email address will give you access to your account anytime. This is the email account where you will receive appointment reminders, new message notifications from your Care Team, and other important information regarding your healthcare.

Yes. All Crossover members can download and use the Crossover app from the App Store or Google Play. Use the same username and password on the Crossover app that you use to log in on the website.

Note for iPhone users: you need the latest version of iOS installed and running on your iPhone as pending iOS updates might prevent the app from opening. Everything you can do on the website can also be done on the Crossover app. With the app, you can opt in to receive real-time notifications from your Care Team and reminders to check in for appointments. You have the option of using a secure, facial- or fingerprint-recognition biometric login— and once you’re logged in, you can easily check your account, send, and read messages, view your appointment and health history, and more.

Wellness and preventive care services are provided at no-to-low cost. Other services may be provided at a charge depending on your health plan coverage. See fee schedule above. Note: Fees for services are collected at the point of service.

You can call 678-377-4080 to reach the wellness center. Additionally, you can message your Care Team through your Crossover account.

You can register your child dependent from the Crossover Platform by clicking on your name in the upper right corner and selecting Account & Settings. Once in this menu, select Dependents. From this screen you can register your child dependent.

If your child is over 12 years old, you will need to invite them to register using their own email address. Once they have registered and verified their email they can begin scheduling care.

You can log in to the Crossover Occupational Medicine site and book an appointment or you can call the wellness center at 678-377-4080.

Appointments are strongly encouraged; however, walk-ins may be accommodated, however, priority will be given to those with scheduled appointments.

All Kaiser Permanente members must create a Crossover account in order to schedule an appointment with a Health Coach at the wellness center. If you need help scheduling an appointment, please call the wellness center at 678-377-4080. If you are a Kaiser Permanente member, visit my.kp.org/gwinnett to schedule an appointment for all of your additional care needs.

Yes. You can instruct Crossover staff to share specific information with your Primary Care provider and other external healthcare providers you see while addressing your overall healthcare needs. The wellness center’s staff may also ask for your permission to share information with your other external providers if they believe it’s medically advisable. You’ll need to sign a Release of Medical Records form in order for them to share your information. This is the same process your Primary Care provider requires to share information outside of their practice.

If you are enrolled in Gwinnett-sponsored health insurance, a referral is not required to see a specialist. Otherwise, your insurance plan may require a referral. When your Crossover Care Team recommends care with an outside specialist or for imaging, the Crossover Care Navigator will help to find an in-network option. It’s always wise to check with your plan to understand any costs for which you may be responsible.

Your Care Team has a thorough understanding of the various health and wellness benefits, programs, and resources available to you as a Gwinnett employee, either directly from Gwinnett or through your medical plan. They are prepared to refer you to these programs as appropriate, based on your individual circumstances.

Crossover accepts credit cards, debit cards, and Health Savings Account and Flexible Spending Account-associated cards. Please note cash payments are not accepted. Expenses incurred at the wellness center are treated the same as expenses incurred at any other healthcare facility. Therefore, if you are enrolled in the Health Care Flexible Spending Account or Health Savings Account and you incur an eligible healthcare expense, you can submit the expense for reimbursement.

No, Gwinnett will not have access to your health information. Your personal health information is protected by federal privacy and security regulations. Crossover Health complies with HITRUST to protect personal health information, following standards for privacy, security, and network architecture. As a separate medical group, Crossover does not share identifiable health information with your employer. Any violation of this privacy policy by your employer or any other person is a violation of HIPAA legislation and carries significant legal and financial consequences.
