[Company Name] is now partnering with Crossover Health to provide you with new and additional ways to achieve your wellbeing goals and earn quarterly incentives. Earning your Wellness Incentive is easy!

 

Self-report your annual physical or biometric screening once a calendar year AND complete one engagement activity each quarter to be eligible for the $100 quarterly incentives and $400 bonus for fulfilling all the requirements in 2024.

Instructions

To receive your full annual incentive bonus, you will need to complete an annual physical or biometric screening in addition to four quarterly wellness activities.

Annual Engagement Activity

Complete a one-hour annual physical, which includes a biometric screening, or 15-minute biometric screening between January - September*, and report that you have completed this activity here.

*Biometric screenings held October-December will be reserved for newly hired associates throughout the year to ensure there are adequate opportunities for everyone.

 

Associates with access to the On-Campus Health & Wellness Clinic

Log into your Crossover account at care.crossoverhealth.com and schedule your annual physical, which includes a biometric screening OR “biometric screening event” only.

Once you have completed your appointment, self-report it here.

Complete your biometric screening by logging into your Crossover account, click “Start care now” select “I need a doctor” and then “I’d like a Preventive Health Assessment (PHA)”. Your care team will help connect you to a local LabCorp for biometric assessments and then review your results with you.

*The Wellness Incentive is paid on a quarterly basis. To be eligible to receive the $100 quarterly incentive, you must:

  • Complete one eligible activity per quarter as shown here on the [Name of Incentive Program] Microsite
  • Self-report your quarterly activity to Crossover Health via the Google Docs form and save the email confirmation each time.
  • Be actively employed on the first day of the following quarter.
    Example: to receive the 1st quarter incentive, you must be actively employed on April 1st.

To be eligible for the annual incentive bonus of $400, you must:

  • Complete all four quarterly incentives. Self-report them on the Google Docs form.
  • Complete an annual physical or biometric screening as described here on the [Name of Incentive Program] Microsite; Self-report this on the Google Docs form.
  • Be actively employed on the first day of the following year.
    Example: to receive the annual incentive, you must be actively employed on January 1 of the following year.
