Evan Richardson
Evan Richardson is the Founder and CEO of FORM, a virtual obesity medicine clinic committed to expanding access to evidence-based obesity care. Evan founded FORM in 2019 when he recognized an opportunity to leverage technology and pharmacological innovations to deliver evidence-based care that could address the obesity epidemic, one of healthcare’s most pressing challenges.
Evan was drawn to health tech early in his career after working as a healthcare investor and seeing the opportunity to do well by doing good. The first company that Evan examined closely was an obesity care service connected to a major diabetes hospital. This experience elucidated how evidence-based medical care could positively impact people living with obesity. Evan was a Healthcare Navigator at Castlight, and later worked as Head of Enterprise Products. He was a member of the founding team at Grand Rounds, a clinical navigator, that later acquired telehealth provider Doctor on Demand to become Included Health. During his time at Grand Rounds, Evan held a range of leadership roles across product, data science, operations, finance, and corporate development.
In addition to leading FORM, Evan currently serves on the board of Bicycle Health, a virtual treatment provider for opioid dependence. Evan has an undergraduate degree in Computer Science from Dartmouth University and an MBA from Harvard Business School. He lives in Boston with his wife and two children.
